PLEASE READ CAREFULLY. BY ACCESSING OR USING THIS WEBSITE, CREATING AN ACCOUNT, OR PURCHASING A TEST KIT, YOU AGREE TO THESE TERMS.

1. Who We Are

This website is operated by DNA4HealthLab, LLC (“we,” “us,” “our”). These Terms govern your access to and use of dna4addiction.com, any related pages, and any digital experience we provide in connection with DNA4Addiction™ and other DNA4HealthLab offerings (collectively, the “Site” or “Services”).

2. Eligibility and Use

The Site is intended for individuals 18 years or older.

Adult Use

You may purchase and use DNA4Addiction™ for yourself.

Minor Testing

If you purchase a test for a minor, you represent and warrant that:

  • You are the parent or legal guardian, or have legal authority to consent

  • You provide all required consents prior to testing

  • You are responsible for all information submitted

We do not knowingly collect personal information directly from minors without parental or guardian involvement.

Teen assent may be requested where appropriate.

3. Educational Use Only. Not Medical Advice. Not Diagnosis.

DNA4Addiction™ provides educational genetic insights designed to support awareness and prevention.

  • The Opioid Risk Score (ORS) is an educational signal, not a diagnosis

  • Results do not predict outcomes or determine behavior

  • The four pathways (Stress, Mood, Reward, Drive) are contextual frameworks

Nothing provided through the Site or Services constitutes:

  • Medical advice

  • Diagnosis

  • Treatment

No provider-patient relationship is created.

If you are concerned about immediate risk or safety, seek professional or emergency help.

4. Ordering, Authorization, and Consent

  • Genetic testing requires informed consent prior to activation and processing

  • Consent must be completed before sample submission

  • Some states may require provider authorization

All testing is performed by a CLIA-certified laboratory and, where applicable, a state-licensed laboratory.

You agree to provide accurate and complete information.

5. Sample Collection and Handling

You are responsible for following all collection and shipping instructions.

  • Improper collection or handling may result in delays or inability to process

  • Recollection may be required

Biological samples are:

  • Used only for the requested test

  • Destroyed within approximately 30 days after testing is complete

  • Not stored or used for biobanking

6. Results Delivery and Limitations

Results are based on:

  • The submitted sample

  • Information you provide

  • Current scientific understanding

Limitations include:

  • Sample quality

  • Technical constraints

  • Evolving science

You agree:

  • Not to use results as the sole basis for medical, legal, or disciplinary decisions

  • Not to use results for immediate safety determinations

Results are delivered electronically via secure email or secure digital delivery.
A patient portal may not be available.

7. Account Security

If accounts are used, you are responsible for maintaining confidentiality and all activity under your account.

8. Acceptable Use

You agree not to:

  • Use the Site unlawfully

  • Interfere with systems or networks

  • Attempt unauthorized access

  • Upload malicious code

  • Scrape or harvest data

  • Use automation without permission

9. Intellectual Property

All content, reports, trademarks, and materials are owned by DNA4HealthLab, LLC or its licensors.

Use is limited to personal, non-commercial purposes.

10. Feedback

You grant us the right to use feedback to improve Services, subject to our Privacy Policy.

13. Refunds, Replacements, and Cancellations

Refund and replacement eligibility depends on kit status, activation, and whether a sample has been received or processed.

THE SITE AND SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE.”

We disclaim all warranties, including:

  • Merchantability

  • Fitness for a particular purpose

We do not guarantee uninterrupted or error-free operation.

15. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW:

  • We are not liable for indirect or consequential damages

  • Total liability will not exceed the amount paid for the test

16. Indemnification

You agree to indemnify DNA4HealthLab, LLC against claims arising from:

  • Your misuse of the Services

We may update these Terms at any time. Continued use means acceptance.

19. Governing Law

These Terms are governed by the laws of the State of California.

Any disputes will be resolved in courts located in Orange County, California.
